ABSTRACT

@#Objective    To explore the efficacy of artificial intelligence (AI) detection on pulmonary nodule compared with multidisciplinary team (MDT) in regional medical center. Methods    We retrospectively analyzed the clinical data of 102 patients with lung nodules in the Xiamen Fifth Hospital from April to December 2020. There were 57 males and 45 females at age of 36-90 (48.8±11.6) years. The preoperative chest CT was imported into AI system to record the detected lung nodules. The detection rate of pulmonary nodules by AI system was calculated, and the sensitivity, specificity of AI in the different diagnosis of benign and malignant pulmonary was calculated and compared with manual film reading by MDT. Results    A total of 322 nodules were detected by AI software system, and 305 nodules were manually detected by physicians (P<0.05). Among them, 113 pulmonary nodules were diagnosed by pathologist. Thirty-eight of 40 lung cancer nodules were AI high-risk nodules, the sensitivity was 95.0%, and 25 of 73 benign nodules were AI high-risk nodules, the specificity was 65.8%. Lung cancer nodules were correctly diagnosed by MDT, but  benign nodules were still considered as  lung cancer at the first diagnosis in 10 patients. Conclusion    AI assisted diagnosis system has strong performance in the detection of pulmonary nodules, but it can not content itself with clinical needs in the differentiation of benign and malignant pulmonary nodules. The artificial intelligence system can be used as an auxiliary tool for MDT to detect pulmonary nodules in regional medical center.

ABSTRACT

Granulocyte colony-stimulating factor (G-CSF) is an essential regulator of neutrophil trafficking and is highly expressed in multiple tumors. Myeloid derived suppressor cells (MDSCs) promote neoplastic progression through multiple mechanisms by immune suppression. Despite the findings of G-CSF function in colon cancer progression, the precise mechanism of G-CSF on MDSCs regulation and its blockade effects on tumor growth remains a worthy area of investigation. In this study we observed an overexpression of G-CSF in a mouse colitis-associated cancer (CAC) model, which was consistent with the accumulation of MDSCs in mouse colon tissues. Further in vitro studies demonstrated that G-CSF could promote MDSCs survival and activation through signal transducer and activator of transcription 3 (STAT3) signaling pathway. Moreover, compared with isotype control, anti-G-CSF mAb treatment demonstrated reduced MDSC accumulation, which led to a marked decrease in neoplasm size and number in mice. Our results indicated that G-CSF is a critical regulating molecule in the migration, proliferation and function maintenance of MDSCs, which could be a potential therapeutic target for colitis-associated cancer.

ABSTRACT

Objective To investigate the clinical effect of vagus nerve preserving pericardial devascularization plus subtotal splenectomy in treating portal hypertension with a history of variceal bleeding.Methods The clinical data of 33 cases of portal hypertension with variceal hemorrhage treated with vagus nerve preserving selective pericardial devascularization plus subtotal splenectomy from April 2004 to December 2013 (study group) were compared with that of 34 cases treated with pericardial devascularization plus splenectomy (control group).Results There was no mortality in two groups.The postoperative gastric drainage during the first 72 h were(1 525 ±30) ml in the study group and (2 130 ±40) ml in control group(P <0.05).Portal vein thrombosis developed in one case in the study group and 15 cases in the control group(P < 0.05).Postoperative 3-year recurrent gastroesophageal varices hemorrhage was 15% in the study group and 25% in the control group (P < 0.05).Postoperative 5-year variceal hemorrhage recurrence rate were 28% in the study group and 30% in the control group (P > 0.05).Conclusions Maitaining vagus nerve selective pericardial devascularization plus subtotal splenectomy is of less postoperative complication and lower portal vein thrombosis rate and better patients' survival compared with pericardial devascularizatim plus total splenectomy.

ABSTRACT

Objective To analyze 278 breast biopsy cases guided by mammography and find the characteristic of the most suitable cases.Methods The initial diagnosis of breast cancer of 278 cases were divided into mass type (197 cases) and non-mass type (81 cases) according to the imaging features,the biopsy was performed in 14G core needle guided by mammography and contrasted to paraffin pathology of post operation.Results The cases of biopsy conformed by paraffin pathology were 260,accuracy rate is 93.52 %(260/278).192 cases were diagnosed pathologically in 197 cases of mass type,and 68 cases in 81 cases of non-mass type.The accuracy rates were 97.46 % (192/197) and 83.95 % (68/81) respectively.Conclusion The mass type with clinical and imaging features is suited for biopsy.To avoid missed diagnosis,it is important to frozen during operation for the cases with the negative result in non-mass type.

ABSTRACT

Objective:To establish a GC fingerprint analysis method for identification of volatile oil in ultramicro-powder of Houttuynia cordata from different habitats, then to control the quality sensitively.Methods: GC was used to analyze the volatile constituents of ultramicro-powder of Houttuynia cordata from 12 different habitats;SPDTM-1CapillaryColumn(30m?0.32mm?0.25?m),temperature programming and FID detector were applied.Results: The mutual mode of GC fingerprints was set up and the similar degrees to the volatile oil from of different habitats were compared.Conclusion:The GC fingerprints of volatile oil in ultramicro-powder can be used to identify the Houttuynia cordata from different habitats and evaluate its quality.

ABSTRACT

Objective To explore the puncture technic and method of the transabdominal L5/S1 lumbar diskectomy (TALD). Methods Two cases of cadaver were dissected and L5/S1 intervertebral disks were explored and punctured from different angles in order to probe the proper puncture angle for the needling. The radiographs of the lumbar and sacral vertabral samples of 30 cases were scrutinized and analysed to find the pancture imaging marks of the bones. When performing barium meal examinations, the lumbar and sacral spine of 200 cases were inspected to figure out the opacification of the rate imaging marks. 68 cases TALD were reviewed. Results The most pivotal process of the TALD is the needle puncture should be in parallel direction with lumbosacral interspace and then be pushed into the intervertebral disk. In order to make sure of the puncture point two sticking points should be fixed, on the abdomen and the another at the front edges of L5/S1 intervertebral disks. The five imaging makes of the lumbar, sacral, spine together with the neighboring L5/S1 intervertebral disks are of important help to find the front edges. Conclusion The correct puncture method is very important for performing TALD.

ABSTRACT

Objective: To observe the clinical effect of Sertraline in treatment of dysthymia. Methods:63 outpatients diagnosed as dysthymia according to ICD-10 were randomly assigned to study group (n=33) and placebo group. Randomized double blind control study was done for 8 weeks with Sertraline from 50-200 mg/d. HAMA and HAMD were used to assess the effect.Results:At the end of 8 weeks, 54 patients completed the trial. Patients of both groups had lower HAMD scores than baseline from 2 weeks, and this trend continued through the 8 weeks observation. During this period, HAMD score of the study group declined more significantly than that of the placebo group. At the end of 8 weeks, the score of HAMA of the study group was also lower than that of the placebo group. The side effects of Sertraline were mild.Conclusion:Sertraline is effective in treatment of psychiatric outpatients with dysthymia.
